Welcome to the Kitty City NM cat adoption program. We request the following information so that we can assist you with your cat adoption process. This form and a consultation with a Kitty City representative are designed to help you find the cat most compatible with your lifestyle.


1. Be 21 years of age or older. (under 21 by approval only)

2. Have identification showing your current address.

3. Have the knowledge and consent of your Landlord.

4. Be willing and able to spend the time and money necessary to provide medical treatment and proper care of your adopted cat(s).

Completion of this application does NOT guarantee an adoption of a Kitty City cat. You must have apprval from a Kitty City representative.

It can be difficult when adopters are adding a pet to their existing pets. Often the existing pets need time to adjust to a newcomer. You must be willing to help your pets the best you can to make the adjustment to a new animal in your home.

What would you do if the vet bills were over this amount? * These questions are meant to make you think about what caring for an animal may involve, not to discourage from adopting. One of our rules is that you return your cat(s) to Kitty City should you ever be unable to care for him/her.

If I am adopting a kitten that is too young to spay/netuer at the time of the adoption, I will have the kitten spayed/neutered when the kitten is between 5-6 months of age. *
I will not ever be cruel to or abuse my cat in any way. *
I will keep an identification tag on my cat at all times. *

Kato Foundation's Kitty City makes every effort to assure that our adoptable animals are in good health. It is possible however that undetectable illnesses or conditions may be present or incubating at the time of the adoption. Therefore Kitty City does not make any guarantee as to the age, behavior, breed, health, temperment, or ultimate size of the animal. Neither Kitty City nor it's Directors, employees, agents, volunteers, representatives, or contractors(including veterinarians), shall be held liable or responsible for any claims or causes of action incurred as a result of the provision or restriction of information, services, adoptions, fostering, veterinary care, or by the actions of any animal transfered on the referenced herein.

I(We) agree to release, hold harmless, and indemnify the Kato Foundation's Kitty City, its officials, employees, agents, volunteers, representatives, and contractors(including veterinarians) from any and all damages and liability of any kind that may arise from my adopted animal, the related spay/neuter procedure, and all related matters. We agree to an inspection of my(our) home by a Kitty City representative to determine suitability for the adopted cat upon our request. *

Your personal signature and date will be required on the adoption form at the time of actual adoption process.
